Due to the continuing spread of coronavirus (COVID-19) across the country, all citizens are being ordered or urged by federal, state, and local governments to take extraordinary precautions to slow the spread including, but not limited to, emergency “Shelter in Place” or “Safer at Home’ orders and other social distancing measures (COVID-19 Precautions). I understand the hazards of COVID-19 and am familiar with the Centers for Disease Control and Prevention (“CDC”) guidelines regarding COVID- 19. I acknowledge and understand that the circumstances regarding COVID-19 are changing from day to day and that, accordingly, the CDC guidelines are regularly modified and updated, and I accept full responsibility for familiarizing myself with the most recent updates.
Townsville Montessori School has taken extra measures of requiring, and if needed, will provide, facial mask, shoe coverings, and gloves while touring the facility. Temperature checks will be taken at the front entrance.
Notwithstanding the risks associated with COVID-19, which I readily acknowledge, I hereby willingly choose to participate in in-person touring of Townsville Montessori School. I acknowledge that: (a) I am responsible for taking all appropriate safety precautions; (b) I am not relying on Townsville Montessori School or its representatives to take safety precautions on my behalf, and (c) I am aware and have reviewed the CDC guidelines accessible at https://www.cdc.gov/coronavirus/2019-nCoV/index.html. I represent and warrant that, to the best of my knowledge: (a) I do not have, and have not had in the last 14 days, any symptoms related to COVID-19; (b) I have not been in direct contact with anyone that has been diagnosed with COVID-19, (c) I have been following all recommended social distancing guidelines and (d) I have not returned from any area, state or country from which the State of New Jersey requires self-quarantine for 14 days, or if I have, I have a complete absence of symptoms and have completed the required 14-day self-quarantine. I consent to the disclosure of this information to any person who has permitted access to this location that I have toured in person.
IN SIGNING THIS AGREEMENT, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing COVID-19 IN-PERSON TOUR DISCLOSURE, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; and I execute this Agreement for full, adequate and complete consideration fully intending to be bound by same.
